This mostly happens at higher ranked games, its definitely not common, but it happens sometimes, in same cases bans were lifted, in same they werent. Its not that bad in overwatch, mostly because of how game works, but i can imagine it can get real bad in game like diablo 2 where players can kill each others in hardcore for example and you can find people more easily.
